Here is the way to fill it in: (I already filled in one activity.)
You should write your name in the following fashion: family name , (yes, put a coma there) given name(s). This is the standard way with the IB. The date must be of the following format: dd.mm.yyyy (where dd is for days, mm for months, yyyy is for the year).
